When you submit this form, you may pay online via the SMG website (credit cards accepted), or download PDF of this form and drop it off or mail it with your payment to the Parish Office (5313 Flad Ave., Madison, WI 53711).

Once your payment has been received, your name(s) will be added to the registered guest list; no paper tickets are necessary!
